In the days after a water main break in southwest Detroit flooded homes and forced residents out on rafts, Jackie Muscat thought something was a little fishy with what she was being asked to sign: a liability waiver that had to be completed if she wanted city-sent contractors to fix her home.

An online damage report form is available for Detroit residents and property owners who were affected by the massive water main break Monday. The Detroit Water and Sewerage Department has posted the link on its site.

Crews were working to restore utilities and assess damages after a water main and secondary line break affected many homes in southwest Detroit.
